DECLARATION UNDER PENALTY OF PERJURY

PURSUANT TO 28 USCA 1746

            I, _____ [CONFIDENTIAL]_____________, declare and state as follows:

4.   [I was abused at Four Winds Hospital at the Katonah, NY campus (They have one in Saratoga, NY, too). Four Winds is a psychiatric hospital and the majority of their populations of patients are minors. I was there in 2005 for the second time in my life (2005 was when the major abuse occurred) and I was having a very hard time, so they had seven people on me- some were restraining me, while others physically beat me. After a while, they injected an overdose of a very powerful sedative, blindfolded me, strapped into a strait jacket and threw me in the back of a van, driving me to a place most of the residents only heard rumors existed. Known as the Annex, it is a solitary confinement building very similar in design to prison isolation cells (concrete walls, large fluorescent light, very high door, one basement style window bolted shut, a tile floor, a surveillance camera and a single exercise mat to sleep on. I was kept in there for 36 hours straight with no food, water or medication.

I have also been in their day program, known as the “PARTIAL” program. Although there was no physical abuse received in my short time in this program, they were extremely nasty to the residents. In addition, academic education for both the inpatient and PARTIAL program patients is for 90 minutes per day, Monday through Friday and kids, even in middle and high school, are taught academics equivalent to a third grade level curriculum or lower.

At the time, one of the diagnoses I had that was extremely prevalent was Asperger Syndrome. However, in defiance of the many doctors I had at home, they swore I did not have Asperger Syndrome, but PDD N.O.S. (Pervasive Developmental Disorder, Not Otherwise Specified). This facility is very stubborn and maintains a messiah complex with the parents, too.

                 In addition, they raise meds EXTREMELY fast here. They told my parents that "they were the professionals and knew what they were doing." This place is hell. Other students have been kept in the annex even longer than I. At least in prison, they get food, water and medication! Bottom line: DO NOT PLACE YOUR CHILD HERE FOR ANY LENGTH OF TIME! They may be affiliated with the Albert Einstein College of Medicine and say they commit to proper care and clinical excellence, but proper care does not exist at Four Winds and the chief psychiatrist who was in charge of my care had such a heavy accent, it was hard for me to understand him during my stays when I was younger. The chief psychiatrist, named Dr. Gonzalez, remains there today (as far as I know).

                 It has taken me a long time to get to the point of recovery in which I can write this account of my experiences. I have been diagnosed with Post-Traumatic Stress Disorder (PTSD) as a result of my experiences at Four Winds Hospital and have continuous nightmares of those experiences. The only way I have finally started to recover is I have started a very rigorous form of imagery rehearsal therapy.]
